We provide structured, person-centered programming focused on four core pillars:
Our model is designed to fill the critical gap that exists after school-age services end—when many individuals face isolation, limited opportunities, and uncertain long-term support.
Life Skills Training
We provide hands-on, real-world training to increase independence and confidence in daily living.
Program elements include:
- Personal hygiene and self-care routines
- Cooking and nutrition basics
- Money management and budgeting
- Transportation training (including public transit use)
- Communication and social skills
- Job readiness and vocational exposure
Outcome: Participants gain practical skills that increase autonomy and reduce long-term dependence on caregivers.
Sports & Physical Wellness
We promote physical health, teamwork, and confidence through structured recreational and athletic activities.
Program elements include:
- Group sports (basketball, soccer, bowling, etc.)
- Fitness programs (walking clubs, strength training, yoga, pickleball clinics)
- Motor skill development
- Inclusive leagues and community-based recreation partnerships
Outcome: Improved physical health, increased confidence, and meaningful peer relationships.
Community Integration
We actively connect participants to the broader community to foster belonging and real-world engagement.
Program elements include:
- Community outings (restaurants, stores, events, monthly movie, lunch bunch, snack chat)
- Volunteer opportunities
- Social clubs and interest-based groups (book clubs)
- Partnerships with local businesses and organizations (Club Thrive and Shine in Faith Ministry located at spaces donated at local churches)
- Life navigation in real-world settings
Outcome: Reduced isolation, increased confidence in public settings, and authentic community inclusion.
Caregiver Support
We recognize that supporting the individual means supporting the family.
Program elements include:
- Respite opportunities
- Support groups and peer networks (Moms United)
- Educational workshops (financial planning, guardianship, future planning)
- Resource navigation and advocacy assistance
Outcome: Reduced caregiver burnout, improved family stability, and better long-term planning for participants.
